MITO Learner Pre-Enrolment Form

Welcome to the MITO learner pre-enrolment form. This form will take approximately 15 minutes to complete. We recommend that both you and your employer complete this form together as it requires information from both parties.

Alternatively, if you are an employer that already has access to the MITO Portal you can enrol an employee via the Portal.

Required documentation

You will need to upload a copy of your citizenship documentation to support your enrolment. Please ensure you have this ready before completing this form.

This should be one of the following supplied in a PDF or JPEG file format:

NZ Certificate of Citizenship.

If you are not a New Zealand citizen, you will need to upload a copy of your Passport, Work Visa and/or Residents Permit where applicable.

Tertiary education may be fees free for eligible first time tertiary students. To determine your eligibility, please visit the Fees Free website before completing this form - https://www.feesfree.govt.nz.

Privacy Statement

The information you are providing to MITO will be used for the purpose of enrolling you into and administering your MITO training programme, and for statistical purposes. We will keep your information secure, and will only share it with other parties that are involved in the funding, administration or delivery of your MITO training programme. If any of the information requested is not provided, we will be unable to process your application. Your information will be retained by MITO in accordance with the Privacy Act 2020. You can request a copy of your information at any time and ask for it to be updated. If you have any questions, please contact MITO on 0800 88 21 21.

Internet Access

MITO learners will need access to the internet to log onto their MITO Portal. Some programmes require learners to complete their practical assessments and/or eLearning and access their goal sheets online - learners will need suitable devices to complete this.

Tertiary education may be fees free for eligible first time tertiary students. To find if you may be eligible go to the Fees Free website - www.feesfree.govt.nz

To be eligible for fees-free for an apprenticeship or training programme in 2019, you must:

be enrolling in a New Zealand Apprenticeship or other programme of at least 120 credits that starts in 2019, is funded by the Tertiary Education Commission, recognised by the New Zealand Qualifications Authority, and is at New Zealand Qualifications Framework (NZQF) level 3 or above; and
be entitled to work in New Zealand; and
have either:
a) been enrolled in a school in or after 2017, other than as an adult student, or
b) not undertaken more than half a year of equivalent full-time tertiary education (0.5 EFTS or 60 credits) at level 3 or above on the NZQF, including tertiary education at an equivalent level undertaken in any country.

The following credits must not be included in measures of prior tertiary education to determine eligibility for fees-free:

any tertiary education undertaken while enrolled in school prior to 1 January 2019 (except for tertiary education undertaken as an adult student), and
any tertiary courses undertaken as part of the student’s or trainee’s school learning programme or secondary-tertiary programme on or after 1 January 2019, and
any credits achieved as part of any industry training programme that consists of fewer than 120 credits that are reported after 1 January 2018.

We understand the eligibility criteria above and have visited the fees free website www.feesfree.govt.nz and can confirm:

1. Yes, we are eligible for fees free (i.e. my status is “YES” on the fees free website and I am enrolling in an eligible programme, and I was not enrolled in a similar programme in 2017).

2. No, we are not eligible for fees free (i.e. my status is “No” on the fees free website).

3. Unknown (i.e. my status is showing as “unknown” on the fees free website) and we are not submitting a “Statutory Declaration” to Fees Free and expect to be invoiced for our training fees.

4. Unknown (i.e. my status is showing as “unknown” on the fees free website) and we have completed the “Statutory Declaration” application and this has been sent to the TEC for approval. A copy of this application is attached to this form.

We confirm there is an employment agreement in place. This is a requirement for signing into a MITO Training Agreement. *

We understand that MITO charges a training fee and the invoiced party will be the employer or the learner. The training fee is to be paid by the employer upon signing the MITO Training Agreement (terms and conditions apply). *

We understand that the learner will be requested to complete two online assessments on the Literacy and Numeracy Assessment Tool and that the completion of these assessments is a requirement for enrolment with MITO. Instructions will be sent via email. *

As an employer, we confirm that the Learner will be actively supported by us in their MITO Training Programme to ensure that they complete their programme within duration and achieve the expected credit achievement. *

We understand that MITO programmes include eLearning modules and that the learner requires access to a device with internet connection to complete these. *

We confirm that the core functions of the learner's position enables them to gain the practical skills and evidence to meet the programme requirements. *

We understand that the learner will be expected to follow all provider guidelines to access workshop(s), off-job training and/or assessments. *
