Use this form when you are sending assessment materials to MITO for moderation.
Moderation Submission Cover Sheet – Registered Assessor
Complete and sign the form and attach it to the assessment materials for each standard you are submitting for moderation.

Report on moderation of assessment decisions
This is the template of the report you receive on your decisions as an assessor.
Moderation Outcome Report – Post-assessment Moderation – Registered Assessor
The report is made by the peer moderator and MITO. It is the outcome of our moderation of the assessment decisions you bring to a cluster group moderation meeting.
The completed report is posted to you.
Appealing a moderation outcome
You may disagree with our moderation of your assessment decision. This is the form you use if you want to appeal the outcome of moderation.
Moderation Appeal Form – Registered Assessor
Complete the form and attach it to the assessment material that was moderated and any other evidence supporting your request for reconsideration.
Send the moderation outcome appeal to the Quality Assurance Manager as per the process in the Procedures Manual — Workplace Assessors.
