Sending assessment materials for moderation
Use this form when your organisation is sending assessment materials to MITO for moderation.
Moderation Submission Cover Sheet – Accredited Organisation
Complete and sign the form and attach it to the assessment materials for each unit standard you are submitting for moderation.
Send the Moderation Submission Cover Sheet and the assessment materials to the Quality Assurance Co-ordinator.

The report is completed by MITO and is posted to your organisation’s moderation contact.

This is the template of the report your organisation receives when moderation takes place after assessment. MITO completes the report after moderating your assessment materials and the decisions of your assessors.

You may disagree with our moderation report. Use this form to appeal the outcome of moderation.
